Fees & Policies

Insurance: Most insurances will be billed for you. Out of network coverage is not a barrier. Your health and wellness is our priority. Monthly payment plans are available, as needed, for established patients.

Payments: In-network co-pays and any deductibles required by your insurance company, are due at the time of each appointment. If you have insurance, please be aware of your coverage before you arrive, know what your co-payment will be before your visit. For cost savings to out-of-network patients, flat fee co-pays are charged.

Scheduling: Appointments are normally scheduled on Tuesdays, Wednesdays, and Thursdays between 10:00 AM and 4:30 PM.
The office is closed on all Mondays, Fridays, weekends, and holidays.

Cancellation: Your scheduled time is reserved for you. If you need to reschedule or cancel your appointment, please call the office at your earliest possible convenience, no later than 24 hours in advance. You can be charged up to the full fee of that appointment for no show/no call or late cancellations. INSURANCE COMPANIES DO NOT ABSORB COSTS INCURRED FOR THIS REASON. Therefore, please keep track of your scheduled appointments.

Prescriptions (for established patients only): Refill requests – it is your responsibility to contact your pharmacy with your request. Be sure to allow 3 business days for your refill request to be processed and available for pick-up at your pharmacy.
